Can you guess what caused this failure?

Root causes:
  1. Someone reloading who shouldn't.
  2. Shooting unknown reloads.
  3. Too fast of a powder.
  4. Obstructed barrel and a non-observant shooter.
  5. Pot-metal barrel (is it a Jiminez/Jennings rifle?)
Never seen a tenon split before!
 
I am not fond of guessing games, so I'll wait for more details from the OP. 1st post without details; I do not even know if this happened to the OP.
More then likely at the doc'sgetting color back from turning white as a sheet,then off to the car wash to spray out the inside and self because alittle soap and water not going to do the trick in that MESS finally to the burn barrel to leave them set of draws into a vapor trail.... :p FOR REAL Praying nobody got injuried other then their feelings...
 
Definitely bore obstruction, perhaps a cleaning rod or laser pointer/scope alignment tool?
I have seen similar from a foam ear plug left in the bore just ahead of the chamber.
Why people DO NOT check for bore obstructions is beyond my comprehension.
I check as soon as I take it out of the gun case/bag.
