Can someone help me ID my tripod’s head?

I need to order two quick release plates for my tripod, but I cannot find the model number of my tripod's head. The tripod itself is a Slik U9000, but I believe it's an older model. I ordered a new QRP that said it was compatible with the U9000, but it was way too long and wide. I called camera land, and they pointed me toward the head I already tried, so that wasn't it.

So you don't have any plates we can reference off of? How about a profile picture of the head?

I've still got that U9000 head at home. I can take a photo of it next to the head and a profile shot later today.
