Range book or Log sheet

Does anyone know where to find a range or log book?
Starting out and what to record what the ammo is doing through my gun.
I'm not reloading, but still what to keep track.
SO any downloads or just a copy of one to make my own would be helpful.

I found some really good info searching the net the other day. Millett's website had what I thought was a good article on making range cards and what not. I found it very useful for a new guy like myself.
