Obstruction in barrel or used the wrong powder, inadvertently used way faster powder resulting in obvious severe over pressure situation.

That happens when there is an obstruction in the barrell, meaning some type of debris was left in the barrel.

Did you fire it before that shot? What was the result of that shot?

Or was this the first shot?

Agree. The breech shank is pretty short.
Either defective steel, improperly stress relieved, and/or excessive pressure at the chamber. As in fast burn rate pistol powder loaded into a rifle cartridge. Possibly a bullet lodged in the barrel just forward of the chamber.
