What’s happening? 22-250??

Have you measured the necks before and after seating? Check it at the case mouth and near the shoulder. If you have brass migration, the sleeve may not have enough clearance to let the case expand when the bullet is being seated. Then check the neck diameter of the sleeve. Also check the trim length of the brass to make sure it isn't too long.
 
Do you have another seating die? I'd try a different seating die to see what it does. How many firings on the brass? It's possible that over repeat firings that the necks have grown thicker as brass has migrated from the shoulder. Your not using any kind of expander I assume so the outside of the necks is the only thing you are affecting with the bushing die. If the necks have grown thicker there is no longer 1thou neck tension. Maybe measure your new brass with seated bullet and old brass with seated bullet and see if the necks are any thicker. It does seem the bullets are definitely deformed more on the old brass than the new. Perhaps you got a batch of really soft bullets and They are just showing something in your brass that has been slowly adding up for a while.

22-250 is notorious for this. But, I think the same thing would happen with new brass. If the sleeve is the culprit, and I think it is, it will do the same thing with new unfired brass. Mine did. Had a heck of a time figuring it out. Head scratching time!
 
Can we just get an answer to what caused this. It's driving me crazy with all the posts. I wish I could just see the dies myself and as soon as Redding is called maybe there is an answer and we can all sleep tonight! I, like everyone else, especially the owner can't imaging this happening. If he has been loading hundres of cases with no problem then all of a sudden this happens with supposedly no changes in reloading process. My original thought was the neck because the bullet was not going into the case and for someone to put that much torque on the press to collapse the bullet. I also voted on the stem being stuck, but why doesn't the stem push the bullet farther into the case. An wht di dit just happen all of a sudden? Hope we get an answer soon. This one will go into the records!
The only time I ever had a problem with reloading is when i changed something in the process on the single stage or progressive loaders.

I'm looking around to see if I can borrow some other dies to see if my sized brass has any problems with other dies.. As soon as redding inspects my dies I will update with their answer.

if you were a bit closer I'd let you try my 22-250 dies out (Rcbs fl dies)... My 22-250 bbl. isn't even on my Striker right now; the 7-08 bbl. is...

I had my 25wssm do this once when the brass wasn't chamfered/deburred enough and the brass had to much carbon in the necks seating Sierra 100 pro hunter pills... Cleaned up the necks and all was well...
