Brass Trimmer slipping

I have a RCBS trimmer that is all od a sudden not holding the case tight and when I start cranking the brass case spins also. Ive tightened all the screws and its still doing it. I have to hold the brass and then turn with the other hand. Has anyone else had this happen. Yes I have the right shell holder on it too. It does it in 270, 30-06, 300 ultra, 22-250. Oh and its not seeming to align straight to the mouth either.
 
Just a couple thoughts:

maybe shell holder is backwards?

might be the spring in there (have never taken it apart to find a spring) is worn out??
maybe oil on the face of where the shell is held down and reduces friction too much?
maybe the cutter is dull and too much resistance to cut so the shell turns?
I'd check with RCBS if you don't get any good advice here!

can you physicall grip the shell and turn it with your fingers? if so, the shell is definitely not being gripped firmly enough (I'd assume a spring issue or the holder is backwards).
