Sounds to me like it needs a good "copper" cleaning.

If I had to guess, I would say that you have a copper build up in the last few inches of the barrel.

Swabbing a barrel is not the same as cleaning it.

Get a good copper solvent (I use Sweets 7.62), a nylon or bronze brush, follow the directions, and scrub the crap out of it. (pun)

If the patches are coming out blue, it ain't clean, yet.

And, it don't cost much.

A 30-06 should be good for between 3500 and 8000 rounds, so, take it from there.

1) Clean out barrel as well as you can to remove all copper and loose surface fouling
2) Plug breech tightly and fill up to the muzzle with copper solvent, let soak half an hour
3) Clean again and shoot

My 300WM is a really accurate gun, but only for 15-20 shots and then I have to bore foam it. Before I learned that, I kept shooting trying to figure the problem out and it got past where a simple cleaning would bring accuracy back. The procedure above fixed it.

Many have given you good advice on checking your bore, crown and cleaning.

Now, go over everything else. Is something loose? Start at the top and work your way down. Verify that rings are solid. Bases...solid. Go ahead and lift your barrel and action out of the stock and verify that your stock is still internally sound. Put it back and use a torque screwdriver to remount it (~65 inch pounds on each screw). Check your barrel channel and verify that nothing is touching the barrel from the tip of the forend all the way to the recoil lug.

The next extreme would be to send your scope back to Leupold and have them verify that your reticle is not slipping around on your.
