I have had this problem for a number of years. Due I think to a combination of things; 1) No use of hearing protection in my youth, 2) 37+ years as a locomotive engineer, 3) Radiation therapy for cancer, 4) Nothing between ears, head is an echo chamber. It is severe and sometimes very bothersome but have learned to deal with it. Ear Doctor says nothing can be done. I usually have earplugs with me at all times, even someone clapping their hands is painful. As a result, I insist our grandchildren wear hearing protection around sound 85db and up.
