I had a similar, odd experience, that may be relevant. It wasn't an Accubond, it was an GameKing as I recall. I was atop a haybale, with a steady rest aimed at an unmoving whitetail quartered toward me 125 yards away. I took the two lung shot with which I'd harvested many whitetails previously. Sight picture looked good, maybe an inch higher that where I'd intended (darn heartbeat). The buck dropped at the shot. I could see the entry wound through the scope, about an inch higher than POA.
I scrambled down from the haybale, at which point the buck jumped up and ran off about 75 yards, and then stopped and started looking around. I dropped to prone, cursed the fact that evidently whitetails had developed ballistic vests, and put one through the head.
Dressing it out, I was able to clearly see where the first bullet had struck the chest, deflected off the near rib, and exited through the digestive tract exit -- yep, right through the rear! Darnest thing I'd ever seen.
So, shot placement was correct, maybe you could call that bullet failure -- or just chalk it up to alien intervention.
